Medication management involves much more than filling a pill organizer. During each visit, our nurses assess the client's general health, cognitive status, medication adherence, and any significant changes that may affect safe medication use. When concerns arise, the nurse communicates with the prescribing provider, pharmacy, authorized family representative, and other members of the healthcare team as appropriate.
Medication Management Services
Depending on the client's needs and provider orders, nursing services may include:
- Reviewing and reconciling prescribed medications.
- Organizing medications in weekly pill planners or approved medication systems.
- Administering prescribed oral, topical, inhaled, or other medications.
- Monitoring medication adherence and identifying missed or incorrect doses.

Injectable Medication Services
SHICS nurses can administer prescribed injectable medications when supported by a valid provider order and individualized plan of care. Services may include:
- Insulin administration and ordered blood glucose monitoring.
- Prescribed vitamin injections.
- Anticoagulant injections.
- Other subcutaneous or intramuscular medications authorized for administration in the home.
- Observation for injection-site reactions or other changes following administration.
- Education regarding the medication schedule, storage, supplies, and sharps safety.
Injectable services are provided according to the prescribing provider's instructions, medication requirements, nursing assessment, and applicable clinical standards.
Flexible Scheduling and Care Settings
Medication-management and injectable visits may be scheduled daily, several times per week, weekly, monthly, or at another clinically appropriate frequency. Services may be provided in:

- Another approved residential setting.
