Recover at Home with Confidence

After hospitalization, personalized care at home can be a comforting alternative to recovering in a rehabilitation or skilled nursing facility.

Following a minor elective procedure, assistance from a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) can provide the support you need to rest and regain your independence. After a more complex hospitalization, private duty nursing can help you safely transition home with individualized clinical oversight and coordinated care.

Benefits of recovering at home may include:

  • Reduced exposure to facility-based infections
  • Dedicated one-to-one attention
  • Assistance with bathing, dressing, meals, mobility, and other daily activities
  • Support and encouragement with prescribed rehabilitation exercises
  • Medication reminders, management, and administration within the caregiver's authorized scope
  • Ongoing observation for changes in condition
  • Communication with physicians, therapists, family members, and other members of the care team
  • Greater comfort, privacy, and familiarity during recovery

Recovering at home may not always be presented as an option during discharge planning. However, when clinically appropriate, SHICS can develop a personalized care plan using private duty nursing, HHA/CNA services, or a combination of both.

Even when clients have complex needs, our experienced nurses and caregivers work closely with families and healthcare providers to support a safe recovery at home. Surrounded by familiar belongings, enjoying preferred meals, and receiving attentive personal care, clients can focus on healing and returning to life on their own terms.
